Once your appointment is confirmed, we send you a short form to complete beforehand, so your first visit is about you, not paperwork.
When you get in touch and we have agreed a time, we send you a secure link to your intake form. You complete it in your own time and return it before your first consultation.
It comes back to us privately and is held safely within our clinical records, seen only by the practitioner caring for you. Having it in advance means we can read your history properly ahead of the day, and spend your appointment on you rather than on forms.

On the day, bring any medical correspondence you have: GP or consultant letters, scan or imaging results, any blood test results, and a note of any medication or supplements. If you have nothing to hand, that is fine; we will take a full history with you.
